Access Team

The Access Team provides a range of services to people who are homeless or who are at risk of becoming homeless. The team also provides targeted services to people who are Aboriginal and/or Torres Strait Islander. The goal of the team is to make health services as accessible as possible to our target groups.

The team members include a dietitian, podiatrist, physiotherapist, occupational therapist, clinic nurse, Aboriginal Engagement Worker and an Aboriginal Health Promotion and Chronic Care Worker/ Billabong BBQ Coordinator.

Children, Youth & Families Team

The Children, Youth and Families Team are a multidisciplinary team consisting of Family Support Services, Speech Pathology, Nursing/Midwifery, Counselling, Youth Work and Nutrition. The team provide a holistic service to families.

Chronic Care Team

The chronic conditions team is a multidisciplinary team that is looking at ways to improve access, service delivery and management, for people with long-term conditions. Chronic conditions can encompass any condition or disease that can affect a person’s physical or mental health, over an extended period of time.

Our team currently is trialling a new behaviour -change model of service delivery for chronic disease. It incorporates techniques of self management and health coaching. This involves working on client goals and working with clients, to improve health and lifestyle, by making small behavioural changes.

To help improve our community's health information and prevention, our team conducts Diabetes screening sessions, LIFE – diabetes prevention series and Living with Diabetes courses for people with diabetes.

By improving client’s self management skills, it can lead to better and healthier lifestyles and better management of chronic disease.

Diversity Team

The Diversity team is a multidisciplinary team with practitioners from Podiatry, Physiotherapy, Occupational Therapy, Community Midwifery, Nursing, Casework/Counselling and Refugee Support. It has been established to recognise and foster the diversity and complexity of people’s lives and health needs. It is working towards a service and programs that are inclusive and responsive of factors such as Aboriginality, cultural, religious and linguistic diversity, gender, sexual identity and orientation and socioeconomic status.

Harm Reduction Team

This is part of our broad range of Drug Safety Services. We aim to provide a quality, confidential and non–judgemental health service, and our emphasis is on reducing the harm associated with illicit drug use - to users themselves, their friends and families, and the whole community.

Yarra Community Support

YCS is our PDRSS outreach service. The Yarra Community Support team offers strength’s based ongoing outreach support to people with a severe mental health issue.
