Hospital Admission Risk Programs

The Hospital Admission Risk Program works with people who frequently arrive at a hospital emergency department their condition being a genuine emergency. However, these clients often have complex medical and social needs. The aim of HARP is to reduce the unnecessary use of hospital emergency departments and to provide clients with better support services in the community.

Different HARP programs have a different focus and they are run in conjunction with the relevant hospital. The following programs are part of our current NYCH services:

The Community Asthma Program

The Community Asthma Program (CAP) provides free asthma education and support for young people with asthma and their family. An asthma educator will work with each family and provide information and support to understand asthma, identify triggers, use puffers and spacers, understand their medications and help develop a written asthma action plan. We usually provide home visits, but can meet with the young person and their families at a community health centre or GP surgery. Referrals can be provided by Royal Children’s Hospital, GP’s or self referral.

The program is run in conjunction with the Royal Children’s Hospital, other Community Health Centres and the Division of General Practice.

ALERT Community Care Co-ordinator

This program provides support and case management to frequent Emergency Department presenters who may be affected by homelessness, mental health, disability or drug and alcohol use. The program aims to work with clients that have complex needs to access appropriate services in the local community to provide ongoing care and support. St Vincent’s Hospital is the collaborator.
