COLLINGWOOD SITE
Important Information for patients: CHANGES TO OUR MEDICAL APPOINTMENT SYSTEM
Commencing Monday 24th October 2011 we will be changing our appointment system at our Collingwood site.
These changes are designed to increase the number of appointments we have available for patients.
Currently we have at least 10–15 patients per day who do not come to their appointment and do not call us to cancel the appointment.
This means that we have 10–15 patients per day who could have been seen if these appointments had been made available.
Commencing Monday 24th October 2011 we will no longer have ‘on the day appointments’ or the ‘script session’ – to replace these we will have a daily ‘drop in’ session – no appointment can be made for these sessions.
The session changes daily so you will need to call 94114333 to see what time the drop in session will be held. The current ‘script sessions’ will no longer take place – to get a script you can attend the drop in session.
The Doctor will only be able to see a limited number of people so we want you to arrive 30 minutes earlier than the session time to be placed in the queue.
As these sessions will be rostered daily we cannot guarantee that they will go ahead if the Doctor rostered for this session calls in sick.
Emergency medical attention will take precedence – as is current practice – and the nurse will be able to call on a Doctor if needed.
What this means for you...
- No on the day appointments
- A daily drop in session with no appointment needed – please check dates and times for sessions by phoning 9411 4333 – or see bottom of this page for the days and times
- These sessions will not replace your regular ongoing scheduled appointments with your Doctor.
The drop in session will include:
- Repeat of current scripts (replacing the script session)
- Medical certificates
- On the day review
- Referrals from nursing
- Priority of access clients who need attention on the day
The drop in session will not provide:
- S8 prescribing
- Chronic Care management (excluding acute deterioration) including GP Management Plan and Team Care Arrangements, Health Assessments and Enhanced Primary Care Referrals
- Taxi card, insurance, taxi driving or driving assessment form completion
- Mental Health Care Plans or reviews – a follow up should be arranged with your regular GP
- Not to be used as a general ‘follow up’ for a patient
PLEASE NOTE THE STARTING TIMES FOR THE DROP IN SESSIONS:
Each session runs for 3 hours and will have a maximum of 18 patients to be seen.
Please arrive 30 minutes before the session starts to join the queue.
